The Occupational Medicine office provides medical surveillance examinations and consultations to Ames Laboratory and university employees according to federal and state regulations. The Occupational Medicine office enhances the efficiency of existing safety and health programs and assists in the reduction of workplace injuries and illnesses.
- Medical Surveillance

Medical surveillance examinations help to identify and monitor Ames Laboratory and ISU employees who work under conditions and with materials that have a potential health risk. Some of these hazards include carcinogens, toxic chemicals, noise, lasers, and biological agents. Examinations are done in order to prevent occupational related problems. The history, review of exposures, physical examination, and associated laboratory tests provide an assessment of overall health.

Voluntary New Hire Physical Examinations
All new Ames Laboratory employees are eligible to receive a comprehensive baseline physical examination. The physical examination may consist of the following: health history, physical examination, routine laboratory analysis (CBC, blood chemistry panel, lipoprotein analysis and urinalysis), audiogram, vision screening for acuity, pulmonary function testing, and electrocardiogram. The results of all physical examinations and laboratory testing are confidential.Periodic Physical Examinations
Periodic physical examinations are offered to all Ames Laboratory employees. The frequency of eligibility for these periodic examinations is based upon the age of the employee. For employees 45 and over the periodic physicals may be taken annually; under 45 years of age every 3 years. - Wellness Services

Medical Treatment

All accidents and injuries occurring at work or in the course of employment must be reported to the employee's supervisor as soon as possible (even if no medical attention is required) and in the following ways:
Report By Whom Directions First Report of Injury (FROI) Supervisor or injured employee Within 24 hours via the ISU Incident Portal - See steps below. Contact Environmental Health and Safety at (515) 294-2153 for guidance and assistance, especially when a serious injury or major loss occurs. Accident Investigation Supervisor As part of FROI, be prepared to provide details about the incident. Supervisors are encouraged to obtain written accounts of the accident. Complete the Supplemental Record of Injury or Illness form to gather information. Incidents Resulting in Hospitalization or Fatality Supervisor Within 8 hours - call Ames Laboratory Human Resources (515) 294-5740 and Environment, Safety, and Health (515) 294-2153. Follow these steps to begin a First Report of Injury and Accident Investigation:

4. Begin entering injury information
The supervisor and/or the injured employee are responsible for completing a First Report of Injury (FROI) via the ISU Incident Portal within 24 hours of when the incident was reported. Upon submission of the FROI, the supervisor will receive an email requesting information relating to the Accident Investigation as part of the FROI process. The supervisor is asked to reply directly to the email with the answers to the questions asked within 24 hours of receiving the email.
